- The Southwest Florida Safety Council, Inc. is a non-governmental, nonprofit, 501 (c)3 corporation dedicated to promoting, encouraging and supporting safety in industry, at home, on the highway, and in public places. The Southwest Florida Safety Council, Inc. has been serving Southwest Florida since 1964.
Our mission statement is focused around public service. Education, public awareness programs, and activities are the components we use to promote public service.
DUI REGISTRATION INFORMATION
(DUI Registration and Course Not Available Online)
Enrollees must register for the DUI program in the county where they live, work, or attend school, unless an exception has been granted by DHSMV.

REQUIRED DOCUMENTS FOR ENROLLMENT
You will be required to have the following documents prior to completion of the DUI school and are encouraged to bring them to enrollment if available. You may enroll without most documents, except proof of residency and a photo ID.

REQUIRED ITEMS
• Photo ID
• Proof of residency
• Ticket
• Blood alcohol content readings / statement of refusal
• Arrest affidavit / report (complete)
• All court paperwork (including case number and judge’s name)
DOCUMENT COPIES
Please bring copies, as they are required for your file. Copies made by SWFLSC are $1.00 per page.
MISSING DOCUMENTS
If you do not have the above items, you are required to obtain them from the county clerk’s office where you received your DUI.
IN-PERSON ENROLLMENT
Enrollment must be completed in person at the office you choose.
ENROLLMENT TIME
Please allow 45 minutes to 1 hour for enrollment.
SPECIAL SUPERVISION SERVICES (SSS) PROGRAM
The Special Supervision Services Program is designed to allow certain drivers, who would otherwise be ineligible, to obtain a hardship license from DHSMV. Special Supervision enrollees must register with the DUI program in the county where they live, work, or attend school, unless an exception has been granted by DHSMV.
REQUIRED DOCUMENTS FOR SSS REGISTRATION
• A letter from the Bureau of Administrative Review to begin the Special Supervision Services process
• Photo ID
