STEP 1: Fill Out Our Personal History Form
STEP 2: Complete the DMV Informed Consent
STEP 3: Provide a Copy of Your Court Conviction or DMV Driving Record
STEP 4: Pay the $50 Deposit (Once All 4 Steps Are Completed)
